• Bring photo ID
• Bring insurance card
• Bring completed forms
• Bring applicable copays

Insurance Information

Our goal is to take as much of the stress out of the financial side of things as possible. We will do our best to help determine the details of your insurance plan as it relates to our services. We submit all claims to your insurance company for you, whether they are in-network or out-of-network. We are in-network with the following insurance companies:

• Aetna
• Cigna
• First Choice
• Regence
• Premera

We are also in-network with other insurance companies who contract with Regence, Aetna, Cigna, Premera, and First Choice, so please check with us if you are unsure about network status. Whether you are in-network or out-of-network with us, we recommend that you call your insurance company before your first appointment. Below are a few good questions to ask your insurance provider:

• Does my plan cover Medical Nutrition Therapy or Nutrition Counseling?
• If not, contact us at 206 909-8022 for other options.
• How much is remaining on my deductible?
• Are there any limitations or exceptions to my coverage?
• How much is my copay?
